Chiropractic Care of MN (CCMI) is founded by a group of chiropractors with Thomas Allenburg, DC as first president

2008

Transitions administrative management services to Landmark (claims administration, U.M., credentialing, provider resources)

2014

Wisconsin expansion with CCMI merger and development of acupuncture product named AcuNet

2015

Establishes ChiroCare Centers of Excellence and network receives its first Press Ganey Guardian of Excellence award

2016

CCMI announces new name Fulcrum Health, Inc.

2017

Patricia Dennis is named CEO

2018

In-source Credentialing; launches new company and product logo designs

2019

In-source U.M.; launches QConnect and Provider Portal, as well as rolls out Quality Rewards program to providers

2019

Opioid reduction pilot using Care Connections: Links primary care and ChiroCare Centers of Excellence doctors to manage pain

2022

Fulcrum offers navigation services to include employers

Fulcrum partners with MN RETAIN, a federally funded study

Fulcrum assumes operations of Health Services Management's (HSM's) physical medicine network services
