Care Connections is bringing healing to the community.

In 2025, Fulcrum Health received a grant from the State of Minnesota to help individuals in underserved communities manage their pain using physical medicine instead of potentially addictive pharmacological treatment.

Through this grant, Fulcrum launched a mobile clinic initiative through the Care Connections program to serve those suffering from acute or chronic musculoskeletal (MSK) pain in Anoka, Hennepin and Ramsey counties.

Physicians

Physical medicine treatment you and your patients can trust.

The traditional approach to addressing lower back pain and MSK conditions is severely lacking. Physical medicine treatments can provide a significantly more efficient path to healing.

Why physical medicine?

  • 80% of the population will experience back pain at some time in their life.1
  • Back pain is the third most common reason for visits to the doctor’s office.2
  • Patients with spinal pain who saw a chiropractor had half the risk of filling an opioid prescription.3
  • Opioid use is reduced by 50-55% when chiropractic care is provided.4

Why Fulcrum?

  • 98% of patients are satisfied with being treated by providers in the Fulcrum network.
  • A physical medicine expert that’s been around for over 40 years.
  • Over 2.6 million members are currently served by Fulcrum’s networks.

Physical medicine helps patients with:

  • Chronic back or neck pain
  • Sports injuries
  • Joint pain
  • Arthritis
  • Tendonitis or bursitis
  • Neurological disorders
  • Orthopedic conditions
  • Sciatica or nerve-related
  • Neuropathy
  • Headaches

If you have a patient who you believe could benefit from physical medicine, we invite you to connect them with our Care Connections team. Together, we can help your patients achieve safe, efficient and effective pain relief.

FAQs

Currently, chiropractic care is available at the Care Connections mobile clinic. However, acupuncture may be available in the future.

Fulcrum Health is a physical medicine expert that connects you to nearby health professionals who can quickly and effectively treat spine and musculoskeletal conditions, including muscle and joint pain.

Physical medicine offers ways to effectively manage pain with the goal of avoiding highly addictive, prescribed painkillers. It may also delay or eliminate the need for invasive surgery with an associated long recovery time and large medical bills. Each type of physical medicine has its own unique approach, but all types provide hands-on care:

  • Chiropractic care diagnoses, treats and prevents musculoskeletal disorders by providing adjustments primarily to the spine.
  • Physical therapy helps people regain strength and flexibility.
  • Acupuncture targets specific points on the skin with needles to trigger various functions of the body and engage the body’s natural ability to alleviate pain and improve health.
  • Massage therapy manipulates muscles and soft tissues to improve physical function, reduce pain and stress, and promote relaxation.

The Care Connections mobile clinic will be available starting in fall 2025.
